The charity - which supports over 69,000 people each year to improve physical and mental wellbeing, along with education throughout the North East - will be included on the back of the players' shirts as the Magpies return to UEFA Champions League action for the first time in 20 years.
Darren Eales, Newcastle United's CEO, commented: "Newcastle United Foundation makes a phenomenal difference to the daily lives of people throughout our region and it is fitting that we are able to unveil these special shirts ahead of such a huge home game for the football club.
"Having the Foundation's logo on the shirts throughout our European campaign this season will hopefully inspire more people of all ages to get involved with the charity and the great work that they do. The Foundation's work deserves to be shown on the global stage and we are delighted that the charity is part of our Newcastle United family."
Steve Beharall, Newcastle United Foundation's CEO, said: "We are absolutely thrilled that Newcastle United has chosen to feature Newcastle United Foundation's logo across all UEFA Champions League shirts this season.
"UEFA Champions League football returning to St. James' Park is an enormous occasion for the North East and we are proud to celebrate the club's achievement together on the world stage.
"For 15 years we have supported communities to improve wellbeing and create life changing opportunities for hundreds of thousands of people across the region. This significant commitment from the club will raise further awareness of our work and create positive impact through real and lasting change.
"I want to extend our sincere thanks and gratitude on behalf of all of us at Newcastle United Foundation to Newcastle United and UEFA for their outstanding support and for championing our cause."
